Your primary care manager, team nurses, nurse case manager, behavior health professionals and clinical pharmacists will work together to ensure safe, coordinated care.
Your health care team will work to coordinate your care regardless of the care location.
Your team will communicate with outside sub-specialists and other health care professionals to obtain seamless coordination of your care.

You will receive timely reminders when you are due for tests or follow-up.
Your health care team will work together to help you navigate the health care system and ensure that all of your concerns are addressed.
You will have opportunities to be directly involved in the design and delivery of health care services to your community. After all, it's not patient centered care until the patient says it's patient-centered.

How to Join

Army Medical Homes are open to TRICARE Prime patients. If you would like to join an Army Medical Home:

Please visit your local TRICARE Service Center and complete the TRICARE Prime Enrollment and PCM Change Form.
Click HERE and follow the online instructions.
After your paperwork is submitted, please continue to be seen at your current clinic until you receive notification that your PCM has been changed.
