|How to Join
|Army Medical Homes are open to TRICARE
Prime patients. If you would like to join an Army Medical
- Please visit your local TRICARE
Service Center and complete the TRICARE Prime Enrollment and
PCM Change Form.
HERE and follow the online instructions.
- After your paperwork is submitted,
please continue to be seen at your current clinic until you
receive notification that your PCM has been changed.
