PATIENT ADMINISTRATION DIVISION - RELEASE OF INFORMATION

The Release of Information Section at Ireland Army Community Hospital, USA MEDDAC, is located on the 1st floor near the elevators. Our mission is to protect each and every service member's information while making it as convenient as possible for active-duty soldiers, retirees, family members and civil service employees to obtain copies of their health records when needed. We are here to serve you!

Inpatient Medical Records are stored at our facility for two years. After two years, they are archived at the National Personnel Records Center in St. Louis, Missouri.

Outpatient Medical Records are stored here as long as the patients are being seen here. After three years of inactivity, the records are archived at the National Personnel Records Center in St. Louis, Missouri.

If your records are active and you are requesting a COPY OF YOUR MEDICAL RECORD for:

  1. Your own personal records

  2. A civilian healthcare provider

  3. A provider at another Medical Treatment Facility (MTF)

  4. An attorney or legal office

YOU MUST:

  1. Complete DD Form 2870 (Above)

  2. Make a copy of your military ID or drivers license (Front AND Back)

  3. Ensure that it is readable

  4. Mail or fax both of the above documents to:

If you are requesting a Transfer of your Active Medical Record from:

  1. Ireland Army Community Hospital (IACH) to another MTF

  2. Another MTF to IACH

YOU MUST:

  1. Complete DD Form 877 (Above)

  2. Make a readable copy of your military ID or drivers license (Front AND Back)

  3. Mail or fax both of the above documents to:

Ireland Army Community Hospital does not keep birth or death certificates. Vital Statistics is a function of state governments and the District of Columbia. To obtain a certified copy for a death or birth at Ireland Army Community Hospital, please contact the Kentucky Office of Vital Statistics.

If the birth or death occurred overseas, contact the State Department Passport Services.
